Memorial has made access to your medical records and imaging as easy as possible! Patient privacy is a priority when releasing records, therefore we have a variety of ways to access your records that each require identification before release. Whether you have been a patient at Memorial, speciality clinics, or Memorial Health LCHC, you can access your medical records all in one place!
Electronically Request
Patients or their legal representatives may request access to medical records and imaging information via the form below. Please note: we require the person requesting the form to upload a photo ID. You can find a list of acceptable documents below. Please allow 72 hours for Medical Records to email requested release form. Sending more than one request will delay this process.
